Read More BACKGROUND: Preclinical students often fail to appreciate the clinical relevance of basic sciencesduring the first year of undergraduate medical training, leading them to lose interest in the subject,and preventing them from achieving the desired goals. In order to rectify this gap in the curriculum,Medical Council of India (MCI) in 2011 published a document announcing curricular strategiesincluding Early Clinical Exposure (ECE) to effectively modify the Indian system of education. Lackof proper guidance prevented many institutions from implementing ECE. Since our institution hadrun a similar program of “Clinical Observership” as early as 2001, we were able to implement ECEin an efficient way.MATERIALS AND METHODS: Early clinical exposure was implemented as a structured program,with the participation of 10 clinical departments since 2013. Feedback from the students, soon afterECE and also from the batch of CRRIs, who had undergone this program while they were preclinicalstudents amply support the effectiveness of ECE in its contents and methods of implementation.Manual content analysis was performed on open comments. After reading the responses, they werebroken down to meaning units, and these were then condensed. The condensed meaning unitswere labeled with codes. The codes were grouped into categories. Themes were derived from thecategories.RESULTS: Out of the 70 CRRIs, 52 responded to the questionnaire. All the CRRIs except one saidthat ECE was very helpful during their clinical postings and internship period. They suggested thatthe number of hours of posting should be increased and also reiterated the fact that a greater numberof clinical departments could be included in the program. Though the beneficial effects were felt inall the domains of learning, the most remarkable impact was felt in the affective domain, whereinchanges are not easy to come by.DISCUSSION: Recently, National Medical Council has come out with plans of including ECE inthe syllabus with strict time schedule. It is felt that the faculty will find our experience of running theprogram for the past five years helpful in implementing this program, for the fullest benefit of thepreclinical students.

Read More BACKGROUND: Empathy is regarded as a fundamental personal attribute for in-training and inpractice doctors. Several factors may play a significant role to facilitate or prevent the ability of doctors’empathy toward their patients. The objective of the present study is to investigate the relationship ofempathy with personality traits (extraversion, agreeableness, conscientiousness, neuroticism, andopenness to experience) among undergraduate medical students.MATERIALS AND METHODS: This study was based on a correlational, cross-sectional designand was conducted from January 2020 to April 2020. The sample consisted of 301 undergraduatemedical students studying in different medical colleges situated in Riyadh, Saudi Arabia. Personalinformation form, the big-five personality inventory, and the interpersonal reactivity index scales wereadministered. Obtained data were analyzed by applying several methods such as mean, standarddeviation, Mann–Whitney U test, Pearson’s correlation coefficient, and multivariable linear regression.RESULTS: Regression analysis revealed that personality dimensions accounted for 10–19% ofthe variance in scores of empathy sub-dimensions of perspective-taking, empathetic concern,fantasy, and personal distress. Agreeableness was found to be associated with empatheticconcern and perspective-taking. Openness to experience was found to be significantly related toperspective-taking and fantasy, whereas extraversion was negatively associated with perspectivetaking. Neuroticism had an association with personal distress, empathetic concern, and fantasy.Additionally, no gender difference was observed and students with people-oriented specialtypreferences showed significantly higher scores on empathy sub-domains than technology-orientedstudents.CONCLUSION: This study confirmed that personality dimensions play a notable role to predictempathetic behaviors among Saudi undergraduate medical students. This study would help to designindividualized strategies by incorporating personality dimensions in the training program to enhanceempathetic behavior among medical students.

Read More CONTEXT: Empathy is considered to be backbone of the patient–physician relationship. Theconsultation and relational empathy (CARE) measure is widely used internationally to measureempathy. However, no validated tool is available to gather patient feedback on dentists’ empathyin India.OBJECTIVE: The objective of this study was to explore the reliability and validity of a CARE measureand to assess the factors influencing CARE score and to determine if there was an associationbetween their CARE score and satisfaction of the patient.SETTING AND DESIGN: A cross‑sectional study was done in dental colleges.SUBJECTS AND METHODS: A questionnaire study was carried out among 100 patients from 6dental colleges in Bangalore using validated CARE measure.STATISTICAL ANALYSIS: Internal consistency of items was evaluated by the Cronbach’s alpha,and construct validity was assessed by confirmatory factor analysis. Satisfaction was assessed bya question response on 5‑point Likert scale. Descriptive and inferential statistics were performedwith significance set at 5%.RESULTS: The mean CARE score was 43.80 ± 5.36. Internal reliability was high (Cronbach’salpha: 0.859) and was reduced by the removal of any of 10 items. High corrected item‑total correlationsranged from 0.752 to 0.847. Factor analysis showed a single solution with high item loadings (>0.80).Self‑perception of overall health (odds ratio [OR] = 3.78), relationship with family (OR = 4.61) andfriends (OR = 3.78), and previous dental experience (OR = 16.00) were more likely, whereasdentist‑provided treatment (OR = 0.20), number (OR = 0.07) and dental treatment taken (OR = 0.13),presence of anxiety (OR = 0.03), and fear (OR = 0.05) were less likely to have CARE score. Thesatisfaction of the patient regressed significantly with the relationship with family members (ß = 0.77)and CARE score (ß = 0.21).CONCLUSION: This study confirms the educational opportunity and implementation of CARE indental students. CARE scores among patients varied depending on personal factors and dentaltreatment‑related factors. The satisfaction of the patient was influenced by the relationship withfamily members and CARE scores.

Read More INTRODUCTION AND AIM: Biopsychosocial approach suggests that the individual’s health conditionis partly influenced by the interactions of biological, psychological, and social processes. It alsoconcludes that the clinical method should be patient‑centered, which is based on the physician’sinvolvement in the patient’s world and the physician’s ability to see the illness from the patient’s view.In this study, we aimed to determine the extent to which medical faculty assistant doctors use thebiopsychosocial approach in their clinical practice.MATERIALS AND METHODS: Our study is a cross‑sectional type, descriptive, and analyticalresearch. The sample size consisted of assistant doctors who met the inclusion criteria which wereaccepted to participate in the study between December 1, 2017, and March 1, 2018, at the medicalfaculty hospital of Dicle University. Two hundred and three physicians participated in our study. ThePatient–Physician Orientation Scale (PPOS), The Jefferson Scale of Physician Empathy (JSPE),and a survey study including questions in relation with sociodemographic characteristics wereconducted in our study. The reliability of the Patient–Physician Orientation Scale (PPOS) and JSPEused in our study was tested with Cronbach’s alpha and found to be 0.878 and 0.931, respectively.Questionnaires were administered by a face‑to‑face interview method during interview. Informationon the purpose and method of the study was given.RESULTS: A total of 203 physicians participated in our study. One hundred and forty‑three (70%)participants were male and 60 (30%) were female. According to medical disciplines, eightphysicians (3, 9%) from psychiatry, 21 (10, 3%) from family medicine, 104 (51, 2%) from otherinternal medical disciplines, and 70 (34, 5%) from surgical medical disciplines participated in thestudy. In the first subgroup of the PPOS, which is used in our study, the average score of psychiatryphysicians was 3.67 ± 0.99, family medicine assistant doctors 3.92 ± 0.67, other internal medicaldisciplines 3.98 ± 0.65, and surgical medical discipline 3.83 ± 0.61. In the second subgroup, which isdefined as the care in the same scale, the average score of psychiatry assistant doctors was foundto be 3.20 ± 0.45, family medicine assistant doctors 3.68 ± 0.54, other internal medical disciplines3.49 ± 0.60, and surgical medical discipline 3.68 ± 0.58. The results of the Jefferson Scale used todetermine the level of empathy in our study are found as psychiatry 78.25 ± 14.78, family physician67.71 ± 14.69, other internal medical disciplines 72.03 ± 13.46, and surgical medical discipline70.87 ± 15.24.CONCLUSIONS: The biopsychosocial approach is primarily an elementary approach model inmedical discipline such as family medicine and should be applied to other internal and surgicalmedical disciplines as well. With conducting this study, the awareness of this area will be increasedand related issues in the literature will be enriched.
