Read More BACKGROUND: Assessment of circulating and scrub skills is an important issue in operatingroom (OR) programs. However, there is a lack of well‑designed tools that are specifically developedfor this purpose. Hence, this study aimed to develop and determine the validity and reliability of achecklist to measure the circulating and scrub skills of OR novices.MATERIALS AND METHODS: This cross‑sectional methodological study was conducted among 124OR technology students who were selected during three consecutive academic years (2019–2020 to2021–2022). The developed checklist was evaluated with face validity, content validity (quantitativeand qualitative), construct validity (known‑groups validity), criterion‑related validity (concurrentand predictive validities), internal consistency (Kuder–Richardson 20, KR‑20), and inter‑raterreliability (intra‑class correlation coefficient, ICC). Known‑groups validity was evaluated by comparingthe difference between the checklist scores of first‑semester and third‑semester students usingindependent samples t‑test. Additionally, concurrent and predictive validities were evaluated by ICCthrough measuring the correlation between the total score of checklist and grades of a multiple‑choicetest and two clinical apprenticeship courses, respectively. Data were analyzed in the StatisticalPackage for Social Sciences software.RESULTS: After reconciling the preliminary checklist in terms of face and content validities, achecklist with 17 sub‑scales and 340 items called “Circulating and Scrub Skills of Operating RoomNovices (CSSORN)” was developed. Regarding the known‑groups validity, the third‑semesterstudents had higher scores compared to the first‑semester students (p < 0.001 in most sub‑scales).Besides, the total score of checklist showed a significant correlation with the criteria of concurrentand predictive validities (ICC = 0.64, ICC = 0.72; P < 0.001). The KR‑20 for the entire checklist was0.90 (range: 0.60–0.93). The ICC for inter‑rater reliability was also 0.96 for the entire checklist (range:0.76–0.99, P < 0.001 in all sub‑scales).CONCLUSION: The CSSORN had appropriate validity and reliability to be used for measuring thecirculating and scrub skills of OR novices. To shed light on the findings, further testing of this checkliston larger populations and in different contexts is suggested.

Read More BACKGROUND: The mini‑clinical evaluation exercise (mini‑CEX) is a formative assessment tooldesigned to provide feedback on skills essential to good medical care by observing an actual clinicalencounter. However, the bigger advantage of mini‑CEX is the structured feedback that it provides tothe students as well as the faculty, thus helping them to make better decisions.MATERIALS AND METHODS: This study was a cross‑sectional observational study. Sixteen surgeryresidents volunteered for participation and five professors conducted sessions; hence, 80 mini‑CEXencounters. Seven core clinical skill assessments were done, and the performance was rated ona 9‑point scale (grouped into unsatisfactory, satisfactory, and superior). Immediate feedback tothe residents was given by the faculty. Delayed feedback from faculty and residents regarding theperception of mini‑CEX was taken. Statistical analysis was done using SPSS version 20 and analysisof variance (ANOVA) for inferential statistics.RESULTS: As planned, 80 (100%) mini‑CEX encounters were conducted. Surgery residents showedimprovement that was statistically significant in the competencies of medical interviewing skills,physical examination skills, humanistic qualities/professionalism, and counseling skills. Most of thefaculty (80%) were able to identify the gaps in the knowledge of students and areas of improvementfor their teaching. However, 60% of the faculty felt that it required more effort than traditional methods.The mean time taken by the assessor for observation and feedback to residents was 12.51 minand 5.68 min, respectively. The mean scores of evaluator satisfaction and resident satisfaction withmini‑CEX sessions were 6.04 and 7.49, respectively.CONCLUSIONS: Mini‑CEX improves the learning environment in residency and also leads toimprovement in medical interviewing skills, physical examination skills, humanistic qualities/professionalism, and counseling skills. It is done in the actual patient encounter and hence preparesthe resident better for dealing with patients in the future.

Read More BACKGROUND: Rational pharmacology use and appropriate prescribing are among the key learningoutcomes in medical education. Some medical faculties include rational pharmacotherapy coursein their education programs at different years of education in Turkey. The aims of this study were toinvestigate the differences in effect of rational pharmacotherapy course on short- and long-terms bycomparing two cohorts who attended the course in different clinical years of medical education byidentifying which parameters of prescription items are different among groups.MATERIALS AND METHODS: This quasi-experimental study was conducted in School of Medicine.Participants consisted of 157 students who attended the course in Grade 4 (n = 110, Group A) andGrade 5 (n = 47, Group B). Students were asked to complete a prescribing task both upon completionof the course and 1 year after. The performance in prescribing was determined by prescriptionscoring form. Repeated measures ANOVA was employed to test the intervention effect betweentwo periods. McNemar test was employed to measure the change in each item on the prescription.Point-biserial correlations between each item on the prescription and their scores on the test as awhole were calculated.RESULTS: The mean score of Group A dropped to 59.41 (standard deviation [SD] = 14.06)from 90.43 (SD = 8.90), and the mean score of Group B dropped to 73.37 (SD = 12.56) from83.91 (SD = 10.03). All the prescription components in the scripts of the Group A students worsenedsignificantly, except the “name of drug,” whereas Group B students maintained most of them after1 year.CONCLUSIONS: This study shows that the long-term retention effect of rational pharmacotherapycourse conducted in later years of education is better than the course conducted in earlier years ofeducation, which may be related to the fact that students in later years are more likely to take onresponsibility for patient therapy process in clinical education.

Read More BACKGROUND: Since the purpose of medicine science is health promotion, education of this groupis very important. The use of new evaluation methods is one of the first educational needs. Giventhat many conventional clinical evaluation methods are not able to fully evaluation students in theclinical settings and only evaluate limited information, This study is designed to evaluate the impactof direct observation of procedural skills (DOPS) and mini‑clinical evaluation exercise (mini‑CEX)on nursing students and their clinical satisfaction skills.MATERIALS AND METHODS: This quasi‑experimental study was conducted in 2018 to evaluatethe dialysis unit of the 6th semester nursing students of Babol University of Medical Sciences in Iran.Samples were selected by the census method and then random allocation. Data collection wasperformed using two questionnaires and two checklists to evaluate the clinical skills and satisfactionof the two groups of testing and control. Moreover, the data were analysed with using the SPSSsoftware (version 18, IBM SPSS Inc., Chicago, IL, USA) through the descriptive and analyticalstatistics (Chi‑square test and t‑test).RESULTS: The results showed that the intervention group compared with the control evaluation scoreof 13/73 ± 2/44, 11/74 ± 2/43, P < 0.002, respectively. Furthermore, the mean score of satisfaction ofthe intervention group with the DOPS method compared to the traditional method of the control groupwas 72/50 ± 7/31, 63/48 ± 9/31, P < 0.001, respectively. As well as, the mean score of satisfactionwith the mini‑CEX method of the intervention group compared to the traditional method of the controlgroup was 73/28 ± 7/78, 63/48 ± 9/31, P < 0.002, respectively.CONCLUSIONS: The results of this study showed that students who were evaluated by DOPSand mini‑CEX methods had a higher score of clinical performance evaluation and higher level ofsatisfaction. Nursing professors can improve the learning process and the satisfaction of nursingstudents by using direct feedback on such evaluation methods.
