Read More Introduction: Medical errors in hospitals kill more people every year than AIDS, breast cancer andauto accidents combined. Widespread consensus exists that health care organizations can reducepatient injuries by improving the environment for safety from implementing different alternativesfrom technical and managerial improvements to considering medical record data. Consideringthe preventability of medical errors, the Agency for Healthcare Quality and Research (AHRQ)developed patient safety indicators (PSIs). This study analyzes the PSIs calculated in AlzahraHospital of Isfahan. Materials and Methods: This study was conducted retrospectively using theinpatient medical record data of hospitalized patients in a six month period, from October 2010to March 2011. An experienced team in the fields of medical record, health management andhealth information technology was involved in data reviewing. Based on a prior consultation andreviewing, some PSIs were selected. Indicators were calculated considering AHRQ guidelines.Excel software and hospital information system software were used. Results: Across all studiedmedical records of patients, out of 25,164 discharges, below measures were calculated.‑8 Foreign Body cases (PSI 5) (0.31 per 1000).‑30 Postoperative Hemorrhage or Hematoma cases (PS I9) (2.2 per 1000).‑5 Accidental Puncture or Laceration cases (PSI 15) (0.3 per1000).‑8 Complications of Anesthesia cases (PSI 1) (2.2 per 1000).‑96 Selected Infections Due to Medical Care cases (PSI 7) (3.8 per1000).‑17 cases of Postoperative Wound Dehiscence (PSI 14) (3.7per1000).‑1 Birth Trauma – Injury to Neonate case, and (PSI 17) (1.7 per 1000).‑18 Obstetric Trauma – Cesarean Delivery cases (PSI 20) (40 per 1000) were flagged bystudied PSIs developed by AHRQ.Conclusion: Comparing with the reported rates by other studies and AHRQ study in 2006, allof calculated indicators have inadequate condition; i.e. these are far from empirical estimatedrates. The hospital administrators should be more sensitive to this issue and perform someimprovement programs.

Read More Background: Today, much attention has been paid to the patient role as the central factor in themanagement of their own health. It is focused on the issue that the patient has a more critical rolecompared with the health-care provider in controlling the patient own health. defines health literacyas the degree to which individuals have the capacity to obtain, process, and understand basic healthinformation and services needed to make appropriate health decisions. Objective: The objective ofthis study was to determine health literacy, health status, healthcare utilization and the relationshipbetween them in 18 - 64 years old people in Isfahan. Structure and Design: This study was adescriptive analytical survey, which was conducted on 300 subjects of 18-64 years old in Isfahanwith Multi-stage sampling method proportional to selected sample size. Materials and Methods:For collecting the data, questionnaire adapted from CHAP (Consumer Assessment of HealthcareProviders and Systems) health literacy questionnaire was used. Health status was measured basedon an assessment of the physical and mental health over the past 6 months by 5° Likert scale. Dataanalysis was performed by using SPSS 18, descriptive statistics, Chi-square test and multivariateanalysis of variance. Results: There was no significant correlation between health literacy, healthstatus and healthcare utilization. Utilization was less in the urban area No. 6 of the city. In thebachelor’s degree group, the health status was lower than the other groups in these cases: Olderages, married, women, large family size, undergraduates, and urban area No. 14. Conclusions:Due to the average prevalence of health literacy in 18-64 years old individuals in Isfahan and lowhealthcare utilization, the followings are recommended: Necessity of more attention to the issueof health literacy, improving the physician-patient relationship and community awareness, whetherthrough health promotion programs or media for the optimum use of available resources.

Read More Purpose: Outsourcing in healthcare is a cost-effective strategy that can lead to increase servicesquality. The aim of this study was to determine the types of services that have been outsourcedin educational hospitals in Isfahan and to investigate managers’ view about the impact ofOutsourcing. Design/Methodology/Approach: A descriptive-survey study carried out in 2010.Our samples consisted of 100 educational hospital and treatment deputy senior managers ofIsfahan University of Medical Sciences and 53 usable questionnaires were received. Surveyinstrument main points were the extent to which educational hospital outsource services and theimpact of Outsourcing. Reliability and validity of the questionnaire have been verified. Data areanalyzed with SPSS18 software. Findings: The results for medical-diagnostics services showedphysiotherapy, radiology, and ultrasound that have the highest rate (33%) of being outsourced.Between logistic and administrative activities, housekeeping, and facility engineering maintenanceservices are the highest rate of outsourced (100%) and green space, CSR, laundry, and medicalrecords are the lowest rate of outsourced (16%). In managers’ view, in relation to advantagesof outsourcing, pay more attention to internal and external customers in private sector (57.2%)was the highest. In relation to disadvantages of outsourcing, costs increase for the patients(45.6%) was the highest. In relation to barriers of outsourcing, forgetting the goal of outsourcing(efficiency) (60.6%) was the highest. Conclusion: Finally, managers’ views about outsourcingin health services organizations were rather acceptable, but in their views, there are barriers inimplementation of outsourcing and they are focused on removing the barriers before outsourcing.Fundamental infrastructure developments as making competing market, promoting of cultures,education, modifying the management attitude and approach, and establishing incentive policiesare emphasized for successful implementation of outsourcing.
